Please follow these steps:
Qataris:
Bring the Health Card application form.
Present your passport, ID card or birth certificate for children along with their vaccination card.
Bring a photograph, except if your medical file contains a new photograph.
Pay the registration fee of 50 QR.
GCC Citizens:
Bring the Health Card application form.
Present your passport, ID card or birth certificate for children along with their vaccination card.
Bring a personal photo.
Pay the registration fee of 50 QR.
GCC Visitors:
Present your passport.
Pay the registration fees.
Residents:
Bring the Health Card application form.
Present your passport, ID card or birth certificate for children along with their vaccination card.
Bring a personal photo.

Qataris: passport or personal card and a photograph.
For children their birth certificate with vaccination card.
GCC Citizens: personal card and a photograph.
For children their birth certificate with vaccination card.
GCC Visitors: passport.
Residents (non-nationals of Qatar or other GCC state): passport/ personal card (valid visa) and a photograph.
